Common questions about your visit
Patients are legally required to present a Valid Provincial Health Card (OHIP) at every medical encounter. This ensures accurate billing and identity verification for clinical safety.
Appointment time slots are strictly allocated for one patient only. If multiple siblings require clinical attention, individual slots must be reserved in advance. We cannot accommodate "add-on" patients during a scheduled visit.
Central Park Medical Clinic requires a minimum of 24 business hours notice for any cancellations or rescheduling. This allows us to offer that time to another child on our waiting list. Failure to provide notice will result in non-waivable fees billed directly to the primary caregiver.
We are committed to a safe, respectful environment. Any instance of harassment, verbal abuse, or physical aggression toward our physicians or administrative staff will result in immediate discharge from the clinic's care.
A little paperwork ahead of time means more of your visit is spent together. Fill these out online, or print and sign them before you arrive.
